Leading Factors for Sagging Breasts

A recent study shows that breast feeding is not the most significant cause of breast ptosis, or sagging breasts. The study shows that age, body mass index (BMI), and smoking are more relevant risk factors, in spite of common beliefs that breastfeeding is the main cause.

A common complaint of postpartum San Diego women, drooping breasts are often the main target for plastic surgery. San Diego breast enhancement can help reverse the signs of breast ptosis and reshape the breasts to be fuller and more aesthetically pleasing. Some women opt not to breastfeed after pregnancy due to the thought that it will adversely affect the appearance of the breasts. The study however, serves to correct this common misconception.

The study was published in the September/October 2008 issue of the Aesthetic Surgery Journal and reviewed a number of pre breast surgery patients who had a history of at least one pregnancy. The degree of their breast ptosis was analyzed in correlation to factors including age, number of pregnancies, history of breastfeeding, duration of breastfeeding, BMI, pre-pregnancy cup size, history of smoking, and amount of weight gain during the pregnancy.

The results showed that the greater the age, higher BMI, greater number of pregnancies, larger pre-pregnancy bra cup size, and history of smoking were all very significant factors in the degree of breast ptosis, while breastfeeding, even duration of breastfeeding was not as relevant. So it seems that those nonsmoking post pregnancy women who are younger, of a decent weight, and smaller cup size need not worry as much about breastfeeding in regards to the development of sagging breasts. Others may consider San Diego breast lift surgery if they are dissatisfied with the look of their breasts.

Choosing the Best Breast Size Implant for Breast Augmentation Surgery in San Diego

San Diego cosmetic surgeon, Dr. Maurice P. Sherman, of the Del Mar Cosmetic Medical Center speaks about a common subject in considering breast augmentation surgery among his patients.

“One of the most important variables in San Diego breast augmentation surgery is choosing the size implant that will be used.

Regardless of the type of implant (saline or silicone gel) and the shape (round or tear drop), the size or volume will be the most critical element in providing the desired fullness. Commonly, patients will tell me they wish to be a certain cup size, but don’t really understand how a bra cup size is measured or the variations in cup sizes.

There are several formulas that manufacturers use in creating a certain cup size. Unfortunately, there is no federal regulation or enforcement of any set standard. Hence, different brands will fit differently and even within a certain brand, the stretch of the fabric will alter the fit and size selected. I feel that perhaps bras from some fancy lingerie brands always run smaller and therefore customers may need to purchase a bra with a larger indicated size tag than they would in using more traditionally made bras. Perhaps this has been a good marketing plan for the company.

We have found that educating our patients and involving them in the selection process helps to create happy satisfied customers. First, we explain the concept that the final breast size is related to not only what size implant is used, but also how much breast tissue is present pre-operatively. Many times a patient will tell us that she wants the same size San Diego breast implants as her friend received, although her friend probably developed with a different breast volume than that patient.

Secondly, I try to steer patients into a well proportioned (but not overly proportioned) final result. The very large breasted women will undoubtedly experience more healing problems, and inherent with aging a higher tendency towards drooping of the breast or asymmetry as one breast may become lower than the other. This would lead to revision surgery and perhaps San Diego breast lift surgery. My goal has always been to create an ideal proportional body where normal tailored clothing will fit naturally, but when the patient wants to show off in tight fitting revealing clothes, the fullness and curves will be noticeable. Most patients readily understand this concept and accept the idea that ‘too big can be harmful.’

Finally, we ask our patient to research and bring to our office, pictures of nudes with different breast sizes. One set being the most desired fullness our patient would like to see when looking at herself in the mirror. Other sets of pictures would be too big and too small breast sizes. Using both inflatable sizing balloons during surgery and our years of experience, we consistently select the size needed to produce very close or exact resemblances to those desired pictures. While there is no quick substitute for experience, judgment, and astute aesthetic abilities of the cosmetic surgeon, having our patients involved with the selection process has been a proven successful technique to translate a women’s desire and aesthetic goals into a successful and fulfilling experience.”

Differences Between Subglandular and Submuscular Breast Implants

This is a common question among San Diego breast augmentation surgery patients; whether their implants should be placed above or below the pectoral muscle. San Diego breast implants that are placed over the pectoral muscle are known as “overs,” or “Subglandular” breast implant placement. Breast implants that are placed under the pectoral muscle are know as – you guessed it – “unders,” or “Submuscular” breast implant placement.

It is best to schedule a consultation with your San Diego breast augmentation surgeon first, before deciding which is best for you, however there are many online resources with information on each choice, just to have an idea. It is good to research before hand, and bring questions to your consultation, that way the doctor can answer them then.

With subglandular breast implant placement, the implants are placed directly on top of the pectoral muscle and just behind the breast tissue. Some women prefer these types of implants because they are easier to place by the surgeon, support larger implants, require less recovery time and post-op discomfort, can create more cleavage, and avoid San Diego breast lift surgery for sagging. On the other hand, some women prefer not to get subglandular breast implants because they tend to have a more augmented look, are more susceptible to rippling and capsule contraction, and can sometimes complicate mammogram readings. Usually it comes down to the woman’s preference of look and feel for this type of placement.

With submuscular breast implant placement, the implants are placed beneath the pectoral muscle, and sometimes only partially beneath the pectoral muscle using specific incisions and placement on the surgeon’s part. This type of placement is ideal for those who want to avoid the risks of having the implants ripple, rupture, or bottom out, as well as have more successful mammogram readings. On the other hand, submuscular implants are a more technical procedure, requiring more healing time, more post-op discomfort, as well as the risk of distorting when the muscle is flexed, and less easily creating cleavage, especially if the implants are placed farther apart. What is Revision Rhinoplasty?

San Diego revision rhinoplasty is a procedure employed when further surgery is required on the nose. This type of nose surgery is often required to repair a botched surgery, or correct irregularities or hide scars form previous surgeries. San Diego rhinoplasty today is much more effective than what it was 10 years ago. Certain technologies employed now were not available years ago, when many people first had nose reshaping surgery. Because of a lack of expertise and outdated surgical techniques, many San Diego nose job patients from over a decade ago have experienced weakening, and mutations of the nasal structures – case in point, Michael Jackson. Think back to when he first got his nose job. It may have suited him for a while, but as time progressed, his nose got weaker, and more deformed-looking. This is exactly the type of situation where revision rhinoplasty comes into practicality.

Some important things to think about when considering revision rhinoplasty in San Diego are how the nose looks now, what exactly is the issue or problems that need to be repaired and if they in fact can be corrected, what kind of scarring is involved, and what the expectations of the patient are.

A San Diego cosmetic surgeon who specializes in revision rhinoplasty will be able to address all these issues. He or she should be specially trained in the field, have much experience, and be able to communicate with you how the procedure will be performed, and what to expect before, during, and after the surgery. Your expectations are key. If satisfaction is not achieved the first time, then more extensive surgery will be required. For example, a patient may bring in a photo of a celebrity and demand an identical nose, and later find that this type of nose does not complement their other features and overall facial aesthetic. It is important to talk with your doctor in order to achieve both satisfying and realistic results.

It is rare that a recent rhinoplasty surgery requires extensive revision work, unless something has gone wrong. Sometimes, patients accidentally cause damage to themselves after the surgery, or are for some reason unsatisfied with the end results. In this case, very small irregularities can be fixed fairly easily and inexpensively on an outpatient basis. More extensive work will require more surgery at more costs. That is why it is important to work with your doctor to get the right results the first time.

Breast Augmentation in San Diego: Silicone vs. Saline Implants

Board Certified San Diego plastic surgeon and breast augmentation surgery expert Dr. Maurice P. Sherman often answers many questions related to breast augmentation, enlargement, enhancement, reduction, reconstruction, and San Diego breast lift surgery. Often, the most discussed topic in breast augmentation surgery is what type of San Diego breast implants to choose. The popular choices tend to fluctuate between saline and silicone implants.

Most implants have some form of outer silicone rubber shell, but they are filled either with silicone gel, or saline (the same type of salt water which is found in I.V. fluids). Variations of silicone and saline implants usually have to do with whether they are filled before or during the operation. Some types of saline implants can be adjusted even after the operation.

Saline implants, which were officially approved for San Diego breast augmentation surgery by the FDA in 2000, have some advantages and disadvantages over silicone breast implants which were FDA-approved in November, 2006. While all implants have some risk of slipping or rupturing inside the body, some argue whether saline or silicone implants are safer in this type of situation.

Breast implants which are filled with saline are easily detected if they rupture. Since it is filled with liquid, if it breaks the implant deflates, noticeable almost instantaneously, and the salt water inside simply gets absorbed into the surrounding tissues inside the body.

Some San Diego breast enlargement patients say that in terms of the look and feel of the augmented breasts, silicone gel offers a more natural look and feel because it is more similar in texture to breast tissue. However saline implants can be more easily altered after surgery, depending on the needs of the patient. If a woman decides that her saline implants are too big, she can ask her doctor to simply subtract some liquid, via small valves and a long needle. Silicone implants’ size cannot be changed without another operation and a newly made implant.

The best way to determine which San Diego breast enhancement procedure is right for you is to schedule a consultation with a qualified and board-certified plastic surgeon in your area. Dr. Sherman practices all forms of aesthetic plastic and cosmetic surgery procedures, serving San Diego, La Jolla, Encinitas, and Del Mar, California.

Are You a Good Candidate for Rhinoplasty in San Diego?

San Diego rhinoplasty is a fairly common procedure, surgically reshaping the nose for either aesthetic, reconstructive, or functional purposes, or all three. Good candidates for rhinoplasty, or nose surgery, are those who are unhappy with the appearance, proportion, or certain features of their nose in relation to the face. Many people look to San Diego and La Jolla rhinoplasty in order change their nose’s appearance and thus enhance the overall appearance of their face. Common qualities of an “unattractive” nose include a large “hump” or a bulbous tip. Rhinoplasty can remove a hump and narrow and reform the tip and overall shape of the nose. Even the slightest changes to the nose can drastically affect the appearance of the face.
Many people also look to rhinoplasty to correct certain functional problems with their nose. A deviated septum and other breathing problems are often fixed with rhinoplasty in San Diego. Also, patients who had undergone rhinoplasty decades ago, prior to the surgical technologies available today, have suffered from weakening in the structure of the nose, leading to breathing problems and changes in appearance. This has lead to a surgical procedure called revision rhinoplasty. San Diego revision rhinoplasty surgeons use today’s technology in order to correct the flaws in outdated procedures, strengthening the nose’s structure and appearance from the inside out.
Talk to your San Diego cosmetic surgeon about whether you are a good candidate for rhinoplasty surgery. It is important to choose a plastic surgeon who will be able to speak to you on the many facets of the procedure, as well as its risks and side effects. Your surgeon should be able to advise you on the type of nose that will enhance your facial features, as well as give you the nose that you will be happy with. Ask to see before and after photos of the surgeon’s work, and don’t be afraid to discuss his or her medical background and experience. In the end, your expectations and your happiness is what matters, but you should always be able to get input from the professionals.
